Access your plan member ID card

Your plan member ID card with your member ID can be printed or shown to your health provider.

Your member ID is on the front of the card. If applicable, the back of your card lists the ID numbers for your dependents. The health provider will need the number for the person who is receiving the service.

Note: Entente Plus does not use policy numbers. If a health provider asks you for a policy number, you can use 9999.

Instructions for the mobile app

1. On the Home screen, select the Wallet icon.

2. Select View Card.

Instructions for the web app

1. On the Navigation Bar , select the drop-down menu with your name or initials on it. Select Plan Member ID Card.

You can also find your card from your Account Settings. If you want to print it, select Coverage, then My Member ID, and then select Print My ID Card.
